To make the most efficient use of our time during your evaluation, the following 3 forms are available her for you to complete prior to your first visit.  Completing them prior to your visit allows you to provide your information accurately and to ensure you have read and understood our policies.

If you have an updated version of Adobe Acrobat Reader, you can complete & sign the forms electronically. Please send your completed forms to appointments@truepilatesphysiotherapy.com

We will have copies of the forms available if you would prefer to complete them upon your arrival. Please note that we cannot begin our work until the forms are complete, and we may be occupied with clients if you arrive early. 

Please note, we have a 24 hour cancellation policy. If you cancel your appointment within the 24 hour window or do not show up for your appointment, we will do our best to give your time to another client. However, if this is not possible, you will be charged your full appointment price. We appreciate your honesty and grant exceptions for illness and emergencies.
